How to Find In-Network Doctors With Level Health

Use your Level Health member portal or your network directory to confirm whether a doctor, specialist, hospital, or lab is in network before your visit. This page shows your Level Health provider search options and how to check if a doctor is in network.

Two Ways to Search

Find In-Network Providers

You can use member portal provider search or go directly to your network directory. Both options help you verify in-network providers before care.

Option 1

Search Through Your Member Portal

Your member portal includes a built-in provider search. This is the simplest option because it is already filtered to your specific plan and network.

1Log in to your member portal.
2Select "Find a Provider" or "Provider Search."
3Search by name, specialty, or location.
4Results are automatically filtered to your network.
Option 2

Search Your Network Directory Directly

Look at the front of your Level Health ID card. It displays the name and logo of your provider network. Use the links below to go directly to that network's directory.

1Find the network name on the front of your ID card.
2Locate that network in the directory below.
3Search by provider name, specialty, or ZIP code.
4Any provider listed as in-network there is in-network for your plan.

Look for the network logo on your member card. If the office cannot validate eligibility in their system, have them verify using your card details and direct them to my.levelhealthplans.com/providers or have them call the provider verification line on the card.
